Consent forms will be distributed to each school and will need to be included as part of the registration packet. This will enhance the participation for a successful event and provide parents with all necessary details and contact information.

After consent forms are returned, we will coordinate with the school nurse/representative to determine a date to come to the school.

Arrangements will be made prior to the treatment date about where the team will need to set up the equipment, etc. The only requirement is that we are in a room with electric outlet access.
Step 4: Screening Day

On the appointed date, we will arrive to provide the student with necessary dental treatment. We will send a form home with each child that will explain all the services that were provided to their child. If a dental referral is needed, we will supply that information to the parent with all contact numbers included.

If any children are classified as urgent, we will provide follow up phone calls to the guardians explaining in detail the child's condition and trying to assist them in getting in quickly to a local dentist. If antibiotics are needed, we will try to initiate that with the parents.

In six months, we will return for their follow-up visit and routine cleanings.
what will my child receive?
Depending on individualized treatment needs, your child will receive...
- a comprehensive dental exam
- x-rays
- a dental cleaning
- sealants (what is a sealant? click here!)
- a fluoride treatment (why does my child need this?)
- a "take-home form" explaining all services provided to your child as well as their treatment needs
- a FREE toothbrush!
